Survey on Evaluation of Occupational Therapy in Mental Health

목적 : 본 연구는 정신건강 분야 작업치료사를 대상으로 평가 현황을 조사하고자 한다. 연구 방법 : 정신건강 기관에서 근무하는 작업치료사 29명을 대상으로 온라인 설문조사를 실시하였다. 설문지는 일반적 특성, 평가 현황, 정신건강 분야 작업치료 활성화를 위한 요구사항으로 구성하였다. 결과 : 정신건강 분야 작업치료사가 평가하는 영역은 일상생활활동이 가장 많았으며, 사용하는 평가도구는 간편 정신상태 평정척도(BPRS), 캐나다작업수행평가(COPM), 역학연구를 위한 우울척도(CES-D)가 많았다. 사용하는 평가도구의 사용이유는 기관의 규정 및 명령이 가장 많았으며, 평가 수행 시 발생하는 고충으로 업무와 관련된 직접적인 평가도구 부재 등 평가 자체에 대한 고충과 작업치료 계획에 도움이 되는 평가도구의 수가 미발생과 정신건강 사례관리시스템(MHIS)상 미등록 등의 고충이 많이 나타났다. 작업치료사의 강화되어야 할 평가 영역은 일상생활활동과 작업수행이 많이 나타났다. 결론 : 정신건강 분야 작업치료사의 주된 초점이 일상생활활동과 작업수행에 맞춰져 있음이 드러나 작업치료사의 정체성이 정신건강 분야에서도 나타나고 있다. 하지만 작업치료사가 평가를 수행함에 있어 여러 고충이 나타났다. 작업치료사가 정신건강 분야에서 역할을 제대로 수행하고 정신질환자에게 성공적인 서비스를 제공하기 위해서는 작업치료 평가에 대한 보험수가 미적용의 문제, 시스템 미비 등 평가 수행을 제약하는 요소를 개선하여 작업치료사가 적절한 평가를 수행할 수 있는 환경이 구축되어야 한다.

Objective : This study aims to identify the status of evaluation used by occupational therapists in mental health. Methods : An online survey was conducted targeting all occupational therapists engaged in the field of mental health, and the data of 29 respondents were analyzed. Results : In mental health, the main evaluation targets of occupational therapists were activities of daily living (ADLs). The most commonly used by occupational therapists were BPRS, COPM, and CES-D. The main reason for using such assessments was the regulation and directives of the institution. The main difficulties in conducting evaluation were the unavailability of assessment tools related to occupational therapy, and health insurance and system-related problems. evaluation areas to be strengthened by occupational therapists for mental health were found in ADLs and occupational performance. Conclusion : This study revealed that the main focus of occupational therapists in mental health is ADLs and occupational performance. However, several difficulties appeared when occupational therapists performed evaluation. In order for occupational therapists to properly play their roles in mental health and to provide successful services to persons with mental illness, an environment in which occupational therapists can perform appropriate evaluations should be established by improving factors that limit evaluation performance.
