Andersen 모형을 이용한 고령 장애인의 미충족 의료 경험과 영향 요인

Factors affecting Unmet healthcare needs among older Persons with Disabilities applied by Andersen’s behavioral model

연구목적: 이 연구는 Andersen 모형을 적용하여, 고령 장애인의 미충족 의료 경험과 영향요인을 파악하고자 하였다. 연구방법: 본 연구는 2017년 장애인 실태조사에 참여한 65세 이상 3,323명의 장애인을 대상으로 미충족 의료경험에 영향을 미치는 요인을 분석하였다. 데이터는 SPSS win 21.0 프로그램으로 기술 통계, X2-검정, 다중 로지스틱 회귀를 사용하여 분석하였다. 연구결과: 고령 장애인의 19.0%가 병원에 가고 싶어도 갈 수 없는 것으로 나타났다. 의료 미충족 경험의 주요 원인은 ‘교통 불편’과 ‘경제적 이유’였다. 고령 장애인의 미충족 의료경험에 대한 다중 로지스틱 회귀분석을 수행한 결과 연령, 학력, 주관적 경제상태, 장애유형, 스트레스 지각률, 자살시도변수가 통계적으로 유의한 영향을 미치는 것으로 분석되었다. 결론: 고령 장애인에 대한 실질적 개입에 필요한 보건의료정책 수립 및 실행 위해서는 장애유형, 경제적 상황, 스트레스 관리가 우선적으로 고려되어야 한다. 고령 장애인의 미충족 경험을 줄일 수 있는 실질적인 프로그램과 건강 사업 마련이 시급이 요구된다.

Objective : The purpose of this study was to examine the factors affecting Unmet healthcare needs among older Persons with Disabilities. Methods : This study analyzed factors influencing unmet medical experience among the disabled aged 65 years or older who participated in the 2017 National survey of the disabled data. Data were analyzed using descriptive statistics, X2-test, multiple logistic regression with the SPSS win 21.0 program. Results : It was found that 19.0% of elderly people with disabilities were unable to visit hospitals even if they wanted. The main reasons for experience unmet medical care were ‘inconvenient transportation’ and ‘economic reasons’. As a result of performing multiple logistic regression analysis on the unmet medical experience of elderly people with disabilities, it was analyzed that age, education, subjective economic status, disability type, stress perception rate, and suicide attempt variables had a statistical significant effect. Conclusion : We should consider for practical interventions including health care policy establishment and medical guarantee for the elderly with disabilities, the type of disability, economic status, and stress management should be considered first. Practical programs and projects that can reduce the unmet experiences of the elderly and disabled should be established.
