In adult patients with skeletal Class III malocclusion, orthognathic surgery may be considered as a first treatment option, but if the degree of discrepancy is not severe, orthodontic camouflage treatment can be performed. Skeletal Class III malocclusion is often accompanied with transverse discrepancy, and because of orthodontic mini-implants, orthopedic midpalatal suture expansion can be successfully achieved by miniscrew-assisted rapid palatal expansion (MARPE) in young adults. A 19-year-old man with a skeletal Class III malocclusion, narrow maxilla, severe crowding, and congenital missing of mandibular second premolars was treated with camouflage treatment using MARPE and mini-implant. After orthodontic treatment, the patient showed proper occlusion. Improved lip support also improved the esthetics of the profile.
