Evaluation of Lip Symmetry after Unilateral Cheiloplasty by Rotation-Advancement Principle

Objective: The authors determined the symmetry of the lip after primary cheiloplasty in patients with unilateral cleft lip. Materials and Methods: The subjects included 79 patients with nonsyndromic unilateral cleft, who had undergone cheiloplasty using Millard’s rotation-advancement principle. With postoperative frontal photographs, anthropometric upper lip analyses were performed using an image analysis software. Seven parameters were measured. Postoperative upper lip symmetry was determined by the Cleft Lip Component Symmetry Index (CLCSI). Namely, the ratio of cleft side to normal side were calculated. We considered the outcome was asymmetric if the measurement was more or less than 5% of the value of 100. Additionally, we compared the same parameters between the complete (CG) and incomplete (IG) cleft lip group. Results: In CG, the mean CLCSI values revealed that the lateral lip length, red vermilion height, Cupid’s bow width and nasal floor width were symmetric whereas lip width/length and red vermilion area were deficient. In IG, the lip width and length were deficient, red vermilion height and nasal floor width were excessive, and others were symmetric. There were no significant differences in all parameters between CG and IG. Conclusion: Totally 50% of parameters were symmetric immediately after the unilateral rotation-advancement cheiloplasty.
