매복된 하악 제2대구치의 교정적 직립을 통한 치조골 재형성 유도와 다목적 골격성 고정원을 이용한 심한 총생의 치험례

Bone regeneration after orthodontic uprighting of impacted mandibular second molar and multi-purpose skeletal anchorage for severe crowding relief

하악 제2대구치 매복은 다양한 연구에서 0.06~0.65%의 유병률을 보이며, 주요 원인으로는 인접치의 위치 이상, 공간부족, 과잉치, 치아종 등이 있다.1,2 매복치의 치료는 매복치의 이식, 발치, 수술적 직립, 수술적 노출 및 교정적 직립 등의 방법이 있다.2 이러한 방법들 중에서 매복치의 치주 상태, 치근단 형성 정도, 매복의 심도 등 다양한 요인을 고려해 가장 바람직한 방법을 선택해야 한다. 본 증례에서는 양측 하악 제2대구치의 매복과 상·하악의 심한 총생을 동반한 환자에서 교정적 직립을 통해 매복 대구치를 견인하고 근심측 치조골의 재형성을 유도하였으며4,5, 심한 총생 해소를 위해 상악 제1소구치, 하악 우측 제2유구치, 하악 좌측 제2소구치의 발치 후 적절한 고정원을 준비하여 안모의 악화 없이 양호한 치료 결과를 얻었기에 이를 소개하고자 한다.

Impaction of a mandibular second permanent molar is rare and occurs with an incidence of 0.06–0.65%.1,2 Main causes of 2nd molar impaction include the position of the adjacent teeth, space deficiency, supernumerary tooth, and odontomas. Treatment option for impacted molar include transplantation, extraction, surgical uprighting, and orthodontic uprighting.2 Among these methods, the most desirable method should be selected considering various factors such as the periodontal status, degree of apical formation, and depth of impacted tooth. In this case, we present a patient with bilateral impacted mandibular second molar and severe crowding who was treated with orthodontic uprighting of impacted second molar to induce alveolar bone regeneration.4,5 For crowding relief, we decided to extract both maxillary first premolar, mandibular right second deciduous molar, and mandibular left second premolar, and prepared proper skeletal anchorage. As a result, good treatment results were obtained without deteriorating facial appearance.
