과개교합과 입술돌출을 동반한 골격성 I급 부정교합의 설측교정 치험례

Lingual orthodontics for skeletal Class I with deep bite and lip protrusion

최근 심미적인 관심도가 증가하면서, 설측 교정치료 역시 주목을 받고 있다. 설측 교정장치는 몇 가지 측면에서 순측 교정장치와 다른 특징이 있다. 그 중 하나로 설측에서 적용되는 압하력의 위치와 치아의 저항 중심의 관계를 들 수 있다. 정상적인 치축을 가진 치아의 경우, 설측 브라켓을 통해 전달되는 압하력이 치아의 저항 중심에 가깝게 지나므로 치아의 수직적 조절이 순측 브라켓을 이용하는 경우보다 유리하다. 또한 전치부 브라켓에 교합판을 포함하고 있는 브라켓(Ormco 7세대 설측 브라켓, OrmcoⓇ, Glendora, CA, USA)을 사용하여 과개교합 환자에서 소구치와 대구치의 정출을 허용하면서 과도한 스피 만곡을 해결하여 교합평면을 개선하는 데 도움을 줄 수 있다. 본 증례는 교합판이 있는 설측 교정장치를 이용하여 효과적으로 과개교합을 개선하였고, 양호한 교합을 얻었기에 이를 소개하고자 한다.

Lingual orthodontic treatment has been widely used as there have been an increase in esthetic interest. Lingual bracket has features that differ from labial bracket in several aspects, one of which is spatial relationship between the location of intrusive force from lingual bracket and the center of resistance of the anterior teeth. In the case of a tooth with a normal axis, the intrusive force delivered from the lingual bracket is closer to the center of resistance than that from the labial bracket. Therefore, lingual bracket is more effective for vertical control than labial bracket. Ormco 7th generation lingual bracket (Ormco®, Glendora, CA, USA) has an anterior bite plate. With this bracket, deepbite can be corrected by discluding the posterior teeth, allowing eruption of the molars and premolars, which improves the deep curve of Spee. This case report describes the use of the lingual bracket with an occlusal bite plate to correct deepbite and obtain good occlusion.
