편측 가위교합 및 심한 하악 전치부 치은 퇴축을 동반한 환자의 치험례

Non-extraction treatment of unilateral scissor bite and severe gingival recession at mandibular incisors

목적: 편측 가위교합 및 심한 하악 전치부 총생과 치은퇴축을 보이는 환자에서 발치를 하지 않고 악궁 확장 및 치근이동을 통하여 치료한 증례를 보고하고자 한다. 임상 증례: 본 증례의 19세 남환은 앞니가 삐뚤하고 어금니가 잘 물리지 않는다는 주소로 내원하였다. 가위교합의 해결을 위해 상악은 횡구개호선을 통한 편측 축소, 하악은 설측호선을 통한 편측확대를 진행하였고, 치조골에서 벗어난 하악 전치 치근을 재위치 시키기 위하여 각형 호선상에서 치근 토크를 부여하였다. 교정 치료 완료 후, 양호한 구치 관계 및 바람직한 수평 및 수직 피개를 얻었으며, 하악 좌측 중절치와 측절치의 치근 위치가 개선되었고, 치은퇴축의 양도 감소하였다. 결론: 가위교합 혹은 구치부 협측 반대교합은 골격성 혹은 치성 원인으로 발생하며, 치성원인인 경우에 교정치료를 통해 해결이 가능하다. 치은 퇴축은 여러가지 원인에 의해 발생되고, 경우에 따라 교정치료를 통해 치은퇴축의 개선과 치은이식의 성공률 증가를 기대할 수 있다.

Purpose: Herein, we report the case of a patient with skeletal Class I malocclusion, unilateral scissor bite, severe mandibular anterior crowding, and gingival recession, who underwent or-thodontic treatment, involving arch expansion and dental root movement without tooth extraction. Case Report: A 19-year-old male patient visited the Depart-ment of Orthodontics with complaints of crowding of teeth and malocclusion. To resolve the scissor bite of the right posterior teeth, we used a transpalatal arch for the upper teeth and a lingual arch for the lower teeth. Torque was applied to the TMA rectangular archwire to improve the root axis. Consequently, satisfactory occlusion and desirable horizontal and vertical relationships between the teeth were achieved. The root po-sitions of the mandibular left central and lateral incisors were improved, and gingival recession was reduced. Conclusion: Scissor bite or posterior buccal crossbite can oc-cur due to skeletal or dental factors, and if it is caused by dental factors, it can be corrected through orthodontic treatment. Gin-gival recession has various causes; in some cases, orthodontic treatment can improve gingival recession and increase the success rate of gingival grafts.
