전치부 최대 견인 시 기존 브라켓과 자가결찰 브라켓에 의한 상악 절치의 3차원적 토오크와 연조직 및 경조직 변화에 미치는 영향

Effects of maximum retraction of anterior teeth on third-order torque of maxillary incisors and soft and hard tissue changes by conventional and self-ligating brackets

목적: Conventional 브라켓과 자가 결찰 브라켓을 사용할 때 전치부 치아 이동 양상을 비교하며 이에 따른 경조직과 연조직의 변화에 대한 특성을 비교하는 것이다. 대상 및 방법: 본 연구는 상악 제1소구치만을 발치하거나 하악 소구치와 함께 발치하여 후방 견인한 lip protrusion 환자 78명을 대상으로 하였다. 치료 전 및 치료 후 측모 두부규격방사선 사진을 분석했다. Active clip metal (ACM), ceramic (ACC), passive clip (PCM) 또는 conventional metal (CM) 그룹으로 나누었다. 네 개 그룹 간의 골격, 치아 및 연조직 변수를 비교하고, ΔU 1to SN (o)에 영향을 미치는 변수를 조사했다. 결과: 치료 후 상악 전치 tip (U1t)은 ACM, ACC, PCM, CM군에서 각각 6.63 mm, 6.95 mm, 6.00 mm, 6.55 mm 견인되었다(P>0.05). U1 to SN (o)의 변화는 ACM, ACC, PCM, CM군에서 각각 10.5o, 11.1o, 12.7o, 12.0o였다(모두 P>0.05). Li를 제외한 연조직의 수평적 변화는 네 그룹 간에 유의한 차이가 없었다(P>0.05). ΔU1 to SN (o)는 U1t 견인량(mm)하고만 유의한 양의 선형성이 있었다(P<0.01). 결론: 자가 결찰 브라켓에 사용된 clip의 형태는 상악 전치부 최대 견인 시 전치부의 각도에 영향을 주지 않았다. 최종 치아의 각도는 사용된 브라켓의 종류보다는 전치부의 견인량에 영향을 받는다. 는 것을 알 수 있었다.

Objective: We compared the patterns of anterior tooth move-ment and the characteristics of hard and soft tissue changes when using conventional versus self-ligating brackets. Materials and Methods: This study included 78 patients with lip protrusion, who underwent retraction by extraction of the maxillary first premolars alone or along with mandibular pre-molars. Pre- and post-treatment lateral cephalograms were analyzed; participants were divided into active clip metal (ACM) or ceramic (ACC), or passive clip (PCM) or conventional metal (CM) groups. We compared skeletal, dental, and soft tissue vari-ables among the 4 groups and identified variables affecting ΔU1 to SN (o). Results: The incisal tip (U1t) of the maxillary anterior teeth post-treatment had retracted by 6.63 mm, 6.95 mm, 6.00 mm, and 6.55 mm in the ACM, ACC, PCM, and CM groups, respective-ly (P>0.05). Changes in U1 to SN (o) were 10.5o, 11.1o, 12.7o, and 12.0o in the ACM, ACC, PCM, and CM groups, respectively (all P>0.05). Horizontal changes in soft tissues, except Li, did not differ sig-nificantly among the 4 groups (P>0.05). ΔU1 to SN (o) showed significant positive linearity only with the amount of traction of U1t (mm) (P<0.01). Conclusions: The types of clips used in self-ligating brackets did not affect anterior tooth inclination after maximum retrac-tion of the maxillary anterior teeth. Final tooth inclination was affected more by the amount of retraction in the anterior teeth than by the type of brackets used.
