Set-up & Hold Articulatory Gesture Training for Persons with Cleft Palate Speech - An Application to Korean High Pressure Consonants

Objective: This study is to investigate the effect of Set-up & Hold Articulatory Gesture Training (S&H AGT) on the articulation accuracy of individuals with velopharyngeal inadequacy after cleft surgery. Materials and Methods: The Korean consonant consists of high percentage of voiceless consonants which need high oral pressure. Also, they are three-way constrast, differentiated by their phonatory types, called ‘tense,’ ‘aspirated’ features. This characteristics makes it difficult to pronounce for persons with VPI and requires special attention for the intervention. To answer this therapeutic goal, S&H AGT was designed to set up articulatory gesture of a specific phoneme by using physiological vocalizations or onomatopoeia which is produced at the same articulation place. Next, it trains to hold the gesture long enough to gain control of oral pressure so as to use it appropriately to the phonetic context. In this study, the effectiveness of S&H AGT was examined through articulation test of the two subjects with VPI problems after the intervention. Results: It appears that the articulation accuracy of individuals who received S&H AGT sessions has improved. For subject 1, the percent of correct consonant increased from 72% before treatment to 86%, and for subject 2, it improved from 51% to 91%. Vowel accuracy in word level also improved for both, reaching 100% from the initial 80%, 90% each. Conclusion: Set-up and Hold Articulation Gestures Training, applied to individuals with VPI problems to produce Korean high pressure consonants, has proven to be effective in improving percent of correct consonant by inducing oral pressure formation in a more stable manner.
