강박장애를 가진 성인자녀에 대한 가족치료 사례연구: 가족체계론적 관점을 중심으로

본 연구의 목적은 가족치료 사례에서 나타난 강박장애에 영향을 미친 요인, 가족치료 과정, 치료자의 개입 방법 및 개입의 효과를 탐색하는 데 있다. 본 연구는 청소년시기에 강박장애가 발병하여 만성적으로 강박장애를 겪고 있는 성인의 가족을 대상으로 진행한 가족치료 상담내용을 분석하였다. 본 연구는 사례연구방법을 사용하였으며, 자료분석을 위하여 주제분석방법과 매트릭스를 사용하였다. 본 연구의 결과는 다음과 같다. 강박장애를 일으킨 요인으로 촉발요인, 개인 요인, 가족 요인, 환경 요인이 나타났다. 전반적인 가족치료 과정은 우선 각 가족구성원에 대해 개인상담을 진행한 후, 남매상담, 부부상담, 가족상담을 하는 순서로 진행되었다. 치료자의 개입방법으로는 내담자 욕구에 맞춘 가족의 불안요인 탐색, 원가족으로부터 전수된 부모의 역기능적 의사소통 방식 인지시키기, 자신의 경험 노출하기, 억눌렀던 마음속 응어리진 감정 표출시키기, 가족구성원의 오해 풀어내기, 효과적인 소통방식 실연하기가 나타났다. 개입결과, 가족구성원들이 효과적인 소통방식을 사용하게 되었고, 내담자의 강박증상이 줄어들었다. 본 연구는 강박장애의 요인을 체계론적인 관점에서 촉발요인, 개인 요인, 가족 요인, 환경 요인을 탐색하였고, 강박장애를 가진 내담자에 대한 가족치료 과정과 치료자의 구체적인 가족치료 개입방법 및 개입의 효과를 보여주었다는 데에 의의가 있다.

Objectives: This family therapy case study aims to explore the factors that contributed to obsessive-compulsive disorder (OCD), intervention approaches used by the family therapist and the effectiveness of treatment. Method: This study employed a case study method and used thematic analysis and a matrix approach in data analysis. Results: Contributors to the development of OCD were triggering, individual, familial, and environmental factors. The therapist's interventions included analyzing the familial factors, promoting the awareness of the parents' dysfunctional communication patterns transmitted from the family of origin, helping the client express the repressed emotions, resolving misunderstandings among family members, and practicing effective communication strategies. Conclusion: This study identified the factors related to OCD from a family systems perspective and explored specific family therapy interventions used by the therapist.
