조현병 가족 개입에 관한 지난 30년 국내 연구동향 - 인식론적 지형분석을 중심으로 -

A Systematic Literature Review on Family Interventions for Schizophrenia in Korea Over the Last Three Decades: Focusing on theoretical perspectives

본 연구는 조현병 가족을 대상으로 직접적 개입을 실행한 국내 학술연구의 경향을 파악하고 추후 발전방향 및 시사점을 제시하고자 하였다. 이를 위해 1990년대부터 2023년까지 발간된 학술지논문을 검색하여 총 24편의 분석대상을 선정하였으며, 각 연구가 채택한 인식론적 근거를 파악하고 개입내용 및 주요성과에 대한 체계적 고찰을 수행했다. 연구결과, 소수의 체계적 접근 및 임파워먼트 접근을 제외하고 대다수가 취약성-스트레스 이론을 기반으로 하는 가족심리교육을 통해 가족의 스트레스 경감 및 대처기술 습득에 초점을 두고 있는 점이 주목되었다. 또한 가족개입 원리에 대한 명확한 이해와 적용을 위해, 인식론적 근거와 효과평가에 대한 면밀한 접근의 필요성이 제기되었다. 이러한 결과에 기초하여, 조현병 가족의 다양한 욕구에 적절히 대응하기 위해서는 가족개입의 선택지를 인식론 측면에서 다변화할 필요가 있음을 적시하였다.

Objectives: This study aimed to conduct a literature review on family interventions for schizophrenia and to propose future directions and implications for research and practice. Methods: The authors searched academic journal articles published from the 1990s to 2023 in Korea and selected a total of 24 studies for analysis. We identified the theoretical perspectives adopted by each study and conducted a systematic review of intervention content and key outcomes. Results: The results revealed that, apart from a few systemic and empowerment-based approaches, the majority of interventions focused on family psychoeducation based on vulnerability-stress theory to reduce family stress and enhance coping skills. In addition, there is a need for a close examination of epistemological foundations and effectiveness to ensure a clear grasp and implementation of family intervention principles. Conclusion: Based on these findings, it's crucial to diversify the options for family intervention from a theoretical standpoint to adequately address the varied needs of families with schizophrenia.
