This case report demonstrated the multidisciplinary treatment of an adult patient with Class II malocclusion, convex profile, retrognathic mandible, gummy smile, and chronic periodontitis. The chief concerns of the patient were facial aesthetics and functional rehabilitation. A surgical approach was required in conjunction with orthodontic treatment to improve the severe skeletal discrepancy. After initial periodontal treatment, preoperative orthodontic treatment was performed and the maxillary first premolars were extracted just prior to the maxillary anterior segmental osteotomy, and the anterior segment was surgically repositioned posteriorly. The favorable overall treatment outcomes, including aesthetic facial appearance, pleasant smile, proper overbite, periodontal status and favorable occlusion, were maintained 8 years after active orthodontic treatment.
