This report describes the successful treatment and an 8-year follow-up using miniscrews to improve unilateral scissors bite in an adult patient. We present a case of an adult patient with a hyperdivergent skeletal pattern, retrognathic mandible, and severe scissors bite with extruded maxillary right molars in contact with the mandibular gingiva. Vertical control was successfully achieved using miniscrews in combination with conventional sliding bracket-wires. Due to the prolonged untreated scissors bite, uprighting the extruded and tilted molars could cause occlusal interference, and clockwise rotation of the mandible could further exacerbate the patient’s skeletal discrepancy. To prevent this, miniscrews were actively used from the beginning of the treatment to eliminate the interference and to reduce the reaction of the adjacent teeth. We also considered periodontal tissue problems that might occur during the molar intrusion from the beginning of the treatment. After 25 months of treatment, satisfactory occlusal relationship without worsening of skeletal discrepancy was achieved. During the 8-year follow-up period, a stable and well-aligned dentition without periodontal problems was observed. The results of this case suggest that non-surgical correction with miniscrew is an effective approach for a camouflage treatment of hyperdivergent skeletal Class II malocclusion with scissors bite.
