Background The purpose of this study was to apply learning methods to maximize the efficiency of education in physical therapy, which is a health field. Objectives It was intended to propose the effect of the class design applied through the results of subject achievement, class evaluation, and class satisfaction evaluation for the application of the 3L-learning design. Methods In this study, self-directed learning (SL), pre-learning & problem-based learning (PBL), and subgroup learning (SL) methods were complexly mounted and applied to instructional design. Results As a result of analyzing subject achievement, class evaluation, and class satisfaction evaluation after applying the 3L complex instructional design, the main score of the evaluation results in the area of understanding and convergence ability and inquiry and application ability all showed an achievement rate of more than 90%. In the class evaluation, satisfaction was 4.80 or higher in the areas of class plan use, interest and curiosity induction, teaching method, fairness of task evaluation, and class difficulty. In addition, in the class satisfaction evaluation, satisfaction was 4.9 or higher in the quality of the class, feedback with instructors, question and discussion environment, video content use, and suitability of each team's task. Conclusions The 3L complex class design will help students majoring in physical therapy in the health field to induce self-directed thinking and learning, thereby helping academic achievement and satisfaction with their major subjects.
