The repair of bilateral complete cleft lip and palate (BCLP) is challenging. The purpose of this study was to introduce a primary repair method for BCLP using Fisher’s method with vomer flap and report a case. A four-month-old infant with BCLP underwent presurgical orthopedic treatment using a naso-alveolar molding appliance. Primary bilateral cheiloplasty using Fisher’s method and simultaneous repair of cleft hard palate using vomer flap were performed at the age of eight months. When the patient was 18 months old, the soft palate was repaired with intravelar veloplasty. The surgical outcome of the employed method was acceptable. The lip has been favorable since the primary repair. The philtrum is symmetric with proper length at the age of six years. After the primary repair of the hard palate with vomer flap, the early postoperative healing was uneventful. Soft palate repair had been efficient because of the early closure of the hard palate. Simultaneous repair of cleft lip and cleft hard palate using Fisher’s method with vomer flap can be judiciously applicable for BCLP.
