The newly proposed cardiometabolic index (CMI) and visceral adiposity index (VAI) have been used to diagnose various cardiovascular disease risk factors, but there is a lack of research confirming their usefulness for diagnosing the metabolically healthy obesity (MHO) phenotype in young adults. The purpose of this study was to verify the diagnostic utility of CMI and VAI for the MHO phenotype. The subjects were 3,408 young people who underwent health checkups at general hospitals in Gyeonggi-do from June 2019 to May 2022. Logistic regression analysis was performed to confirm the association between CMI and VAI and the MHO phenotype, and receiver operating characteristic curve analysis was performed to confirm the diagnostic ability of CMI and VAI for the MHO phenotype. The frequency of the MHO phenotype increased with increasing CMI and VAI. After adjusting for confounding factors, CMI and VAI showed a higher hazard ratio of MHO phenotype in the 4th quartile than in the 1st quartile of 14.540 (8.480-24.930, p<0.001) and 3.743 (2.380-5.885, p<0.001), respectively. ROC results showed that CMI and VAI had acceptable diagnostic ability (AUC: CMI 0.789, VAI 0.727). In conclusion, CMI and VAI were associated with an increased probability of developing the MHO phenotype, and were confirmed to be useful indicators for screening the MHO phenotype.
