Synchronous ipsilateral parotid tumors containing amylase crystalloids with cytologic and histologic correlation

Synchronous ipsilateral tumor formation within the salivary glands is rare. In the present case, the patient presented with left-sided otalgia. Ultrasound-guided fine needle aspiration revealed numerous rectangular crystalloids with benign-looking ductal cell nests, suggesting the pres-ence of amylase crystalloids. The surgically resected parotid gland contained two adjacent but separate mass-like lesions, which were histologically identified as pleomorphic adenoma and Warthin tumor. An amylase crystalloid component was identified within the cystic space of the Warthin tumor. To the best of our knowledge, this is the first reported case of a synchronous pa-rotid gland tumor containing amylase crystalloids with cytologic-histologic correlation. The pres-ence of amylase crystalloids in fine needle aspiration specimens is mostly indicative of non-neo-plastic lesions or benign tumors, making it of utmost importance to pathologists. Furthermore, this report aims to remind clinicians of the significance of considering benign conditions, thereby allowing them to avoid unnecessary and excessive interventions by ruling out the possibility of malignancy.
