전치부 총생과 횡적 부조화를 동반한 골격성 III급 부정교합 환자의 투명교정장치 치험례

Expansion and total arch distalization using clear aligners (Invisalign) in an adult skeletal Class III malocclusion

목적: 횡적 부조화와 총생을 동반한 골격성 III급 부정교합 환자에서 투명교정장치(Invisalign)를 이용하여 미소 심미의 개선과 기능적 교합을 달성한 증례를 보고하고자 한다. 증례: 본 20세 남환은 상·하악의 총생을 주소로 내원하였다. 검사결과 경미한 하악 전돌과 하악 치열 중심선의 좌측 편위, 전치부 총생, 상·하악골 횡적 부조화, 우측 III급 구치 및 견치 관계를 보였으며, 이외에도 다발성의 상악 전치, 소구치부 백색 반점이 관찰되었다. 악궁의 횡적 부조화 해결을 위해 miniscrew-assisted RPE(MARPE)를 사용한 상악 악궁의 확장이 계획되었으며, I급 견치 및 구치관계의 형성을 위해 골격성 고정원을 사용한 하악 우측 구치부의 후방이동이 계획되었다. 환자가 심미적인 장치를 원하였고, 구내 다수의 백색 반점이 존재하는 것을 고려하여 투명교정장치를 사용하기로 결정하였다. 치료 결과 총생이 해결되고 효과적인 하악 편측 치열의 원심 치체이동을 통해 중심선이 개선되면서 기능적 교합이 달성되었다. 결론: 골격성 III급, 전치부 총생, 중심선 편위, 횡적 부조화를 동반한 환자에서 투명교정 장치와 적절한 골격성 고정원의 사용을 통해 효과적으로 구강위생을 관리하면서 만족스런 미소심미와 교합의 개선을 이루었다.

Purpose: We report a successful clear aligner treatment of an adult Class III malocclusion with posterior crossbite and crowd-ing. Case Report: A 20-years-old male patient visited the clin-ic with a chief complaint of anterior crowding. Examination revealed mild mandibular prognathism, mandibular dental midline deviation to the left side, anterior crowding, posterior crossbite and Class III canine and molar relationships on the right side. Multiple white-spot lesions were observed on the maxillary anterior and premolar teeth. To improve the occlu-sion, miniscrew-assisted rapid palatal expansion (MARPE) and total distalization using a temporary anchorage device (TAD) on the lower right side were planned. Considering the patient’s preference for esthetic appliances and the presence of multiple white spot lesions, clear aligners were applied. After treatment, ideal Class I occlusion with improved smile was established. Superimposition indicated total distalization of the lower arch along with retraction of the upper anterior teeth. Conclusion: Upper expansion, crowding relief, total distal-ization with midline correction was successfully achieved with clear aligners in Class III malocclusion, while maintaining good oral hygiene.
