다수의 상실치 및 안면비대칭을 동반한 골격성 III급 환자의 선수술 치험례

Surgery-first orthodontic treatment of a skeletal Class III malocclusion patient with multiple missing teeth and facial asymmetry

목적: 다수의 상실치 및 안면비대칭을 동반한 골격성 III급 환자에 대하여 선수술을 진행하여 수술 전 심미적, 기능적 불편감을 거치지 않고 빠른 외모개선을 달성하였으며 수술 후에도 안정적인 악골 관계를 유지하며 치료를 마무리 한 증례를 보고하고자 한다. 증례: 본 18세 남환은 다수의 상실치를 주소로 내원하였으며, 그 외에도 안면비대칭 및 골격성 III급 경향을 보였다. 환자는 심미적으로 빠른 개선을 원하였고 절대적 횡적 부조화를 보이지 않았기에선수술을 시행하였다. 선수술을 시행하였음에도 교정 기간동안 하악골은 예측가능한 정도의 적은 움직임만 보였으며 최종적으로 하악돌출 및 안면비대칭을 해결하고 다수의 상실치로 인해 불균일하게 분포되었던 치아 사이 공간을 정리하여 최종 보철까지 마무리하였다. 결론: 다수의 상실치, 안면비대칭, 골격성 III급 부정교합을 동반한 환자에서 절대적 횡적 부조화의 평가 후 선수술을 통해 빠른 외모 및 기능적 개선을 이루었으며 술후 치료 기간동안 악골의 위치도 안정적으로 유지되었다.

Purpose: We report a case in which surgery-first orthodon-tic treatment was performed in a patient with skeletal Class III malocclusion accompanied by multiple missing teeth and facial asymmetry, achieving rapid esthetic and functional im-provements without the discomfort of presurgical orthodontic treatment. Moreover, a stable jaw relationship was attained after the surgery following treatment completion. Case Report: An 18-years-old male patient presented with multiple missing teeth, facial asymmetry, and a tendency towards skeletal Class III malocclusion. The patient desired rapid esthetic improvement and did not exhibit an absolute transverse discrepancy, leading to the decision to perform sur-gery-first orthodontic treatment. Despite the surgery-first ap-proach, the mandible showed only a predictable degree of min-imal movement during post-surgical orthodontic treatment. Ultimately, the treatment addressed mandibular protrusion and facial asymmetry, and organized the unevenly distributed spaces between teeth due to multiple missing teeth, concluding with fabrication of the final prosthesis. Conclusion: In a patient with multiple missing teeth, facial asymmetry, and skeletal Class III malocclusion, rapid improvements in appearance and function were achieved through sur-gery-first orthodontic treatment after evaluating the absolute transverse discrepancy. Stable jaw position was maintained following surgical treatment.
