MARPE를 이용한 비발치 비수술 교정치료: 이론과 실제

Non-extraction camouflage treatment with MARPE : theory and reality

이전에는 성인에서 심한 상하악궁의 횡적 부조화를 동반한 부정교합의 경우 안정적으로 사용가능한 골격성 고정원이 없어 비수술인 교정치료가 제한적으로 시행되었다. 이 경우 유일한 해결책은 수술을 동반한 구개급속확장장치(SARPE)의 사용이었으나, 대부분의 환자들은 수술의 위험성과 추가적 비용으로 인해 수술을 기피하였다. 최근에는 미니스크류의 사용을 통해 비수술적으로 성인에서 전후방적, 횡적, 수직적 부조화를 개선할 수 있게 되었다. 특히 횡적 부조화의 경우 미니스크류 지지형 구개급속확장장치(MARPE)의 사 용을 통해 추가적인 비용 지출없이 임상적으로 효과적이고, 안정적인 상악궁의 확장을 도모할 수 있게 되었다. 본 연제에서는 성인에서 MARPE를 이용한 비수술적 교정치료의 이론과 실제를 논하기 위해 관련 임상 증례와 논문을 함께 살펴보고자 한다.

In the case of malocclusion with severe maxillomandibular transverse discrepancy in adults, the nonsurgical orthodontic approach is limited owing to the absence of reliable skeletal anchorage. The only treatment was surgically assisted rapid palatal expansion (SARPE). However, most patients are reluc-tant to undergo surgery owing to the associated risks and costs incurred. However, with the recent introduction and use of miniscrews, a nonsurgical orthodontic approach can improve skeletal anteroposterior, vertical, and transverse discrepancies in adults. Miniscrew-assisted rapid palatal expansion (MARPE) can be performed primarily in the transverse dimension; compared to SARPE, MARPE reduces the cost to the patient and achieves clinically acceptable stable maxillary transverse expansion. I would like to discuss the theory and reality of using MARPE to expand the scope of nonsurgical orthodontic treat-ment in adults based on clinical cases and related articles.
