The purpose of this case report is to present an orthodontic treatment of a patient with skeletal Class III malocclusion and maxillary transverse discrepancy. An 18-year-and-10-month-old female patient visited the orthodontics department with an anterior crossbite. She was diagnosed with skeletal Class III malocclusion, showing a hypodivergent pattern accompanied by labially ectopic eruption of the maxillary right central incisor and transverse discrepancy of the maxilla. Her maxilla was expanded by applying a maxillary skeletal expander (MSE), and Class III intermaxillary elastics were used to correct the anterior crossbite and low angle through clockwise rotation of the mandible resulted by extrusion of the posterior teeth. The total orthodontic treatment period was 22 months. Her concave facial appearance was significantly improved, and her anterior crossbite and transverse discrepancy were completely corrected. The maxillary right central incisor, which had erupted ectopically labial, was well aligned. After one year of retention, a stable occlusal relationship, proper overjet and overbite, and good maintenance were confirmed. (Clin J Korean Assoc Orthod 2024;14(3):181-192)
