The article presents a case of spontaneous correction of severe gingival recession and anterior open bite in a 31-year-old female patient after orthodontic treatment. The treatment involved repositioning the root into the alveolar bony housing and self-regulating the tongue-thrusting habit. Cone-beam computed tomography was used to assess the 3-dimensional root position and inclination before and after treatment. The root of the mandibular left lateral incisor was positioned labially and outside the alveolar bone. It was orthodontically moved toward the center of the alveolar bone by applying additional lingual root torque using segmental arch mechanics, followed by the continuous archwire technique. To improve the anterior open bite, the patient was instructed to wear a tongue crib appliance on the maxillary arch to self-regulate the tongue-thrusting habit. After treatment, the root was moved into the alveolar bone, and the gingival recession was corrected. Additionally, the anterior open bite was effectively corrected with minimal use of inter-arch elastics. (Clin J Korean Assoc Orthod 2024;14(3):215-226)
