우리나라 성인 당뇨병 환자의 우울 경험 현황: 사회복지실천에의 함의

Depression among Adults with Diabetes in Korea: Implications for Social Work Practice

당뇨병 환자의 우울 증상은 혈당조절 및 자기관리의 어려움을 유발하여 당뇨병 합병증 발생 위험을 높이고, 삶의 질을 저하하며, 나아가 사망률에도 영향을 미친다. 특히, 당뇨병 환자들에게는 임상적으로 의미 있는 수준의 우울뿐만 아니라 우울감의 증가도 부정적 결과를 초래하는 것으로 나타나 당뇨병 환자의 건강 및 삶의 질 향상을 위해 우울은 중요하게 다루어질 필요가 있다. 본 논문은 다양한 실천 현장에서 당뇨병이 있는 클라이언트에게 서비스를 제공하는 사회복지사들의 당뇨병에 대한 이해를 높이기 위하여, 관련 문헌을 검토하고, 우리나라 성인 당뇨병 환자의 우울과 관련된 현황을 파악하는 것을 목적으로 한다. 구체적으로, 당뇨병과 우울의 동반이환에 대한 가설 및 당뇨병 환자의 우울 증상과 관련된 특성 등을 정리하고, 2023년 지역사회건강조사 자료를 활용하여, 우리나라 19세 이상 성인 당뇨병 환자(n=31,725)의 우울감 및 우울 증상 경험, 정신건강 서비스 이용 현황 등에 대해 살펴보았다. 이를 바탕으로 다양한 사회복지실천현장에서 당뇨병 클라이언트를 만나는 사회복지사를 위한 실천적 함의를 논의하였다.

Depressive symptoms in patients with diabetes can lead to difficulties in blood sugar control and diabetes self-management, increasing the risk of diabetes-related complications, lowering quality of life, and even affecting mortality. For diabetic patients, not only clinically significant depression but also an increase in depressive symptoms can result in adverse outcomes, highlighing the importance of addressing depression for health and well-being among adults with diabetes. This paper aims to enhance social workers' understanding of diabetes, especially for those providing services to clients with diabetes in various settings, Specifically, this paper examines the hypotheses regarding the relationship between diabetes and depression, explores characteristics related to depressive symptoms in diabetic patients, and analyzes data from the 2023 Community Health Survey to investigate depressive symptoms and the use of mental health services among adults with diabetes aged 19 and older in South Korea. Based on these findings, practical implications for social workers working with diabetic clients in diverse social work practice settings are discussed.
