진행성 암 29세 청년의 자기실현 사례연구: 모래놀이 상담으로 완화치료

A Case Study of Self-Actualization in a 29-Year-Old Man with Advanced Cancer: Palliative care through Sandplay Counseling

본 연구는 진행성 암(대장암 4기) 진단을 받은 29세 남성의 사례이다. 내담자는 암 진단 후 수술과 항암치료, 죽음에 대한 두려움 등 불확실성에 대한 불안으로 상담을 오게 되었다. 이론은 융의 분석심리학에 기반한 자기실현 (개성화 과정)을 적용하였다. 자기실현의 4단계는 개인이 자신의 내면세계를 이해하고, 진정한 ‘나’를 찾아가는 것이다. 첫 단계(6회기)는 ‘페르소나와의 구별’이고, 두 번째 단계(7-8회기)는 ‘그림자의 의식화’이다. 세 번째 단계(9-13회기)는 ‘아니마와의 융합’이고, 네 번째 단계(14-15회기)는 ‘자기실현(개성화 과정)’이다. 내담자는 죽음 시한부 3개월 앞두고 모래 상자 마지막 15회기에 대극을 통합하여 하나의 빛의 세계로 들어갔다. 본 연구에서 내담자는 모래놀이 상담을 통해 죽음에 대한 불안과 두려움이 완화되어 죽음을 자연스럽고 편안하게 받아들이고 마음을 잘 정리하고 하늘나라에 갔다. 내담자는 자기실현의 전체성으로 “죽음과 결혼하는 신성혼”으로 끝을 맺게 되었다는 것에 의미가 있다.

This study is a case of a 29-year-old male diagnosed with advanced cancer (stage 4 colon cancer). The client came for counseling due to anxiety about uncertainty such as surgery, chemotherapy, and fear of death after being diagnosed with cancer. The theory applied self-actualization (individualization process) based on Jung’s analytical psychology. The four stages of self-actualization are for an individual to understand his or her inner world and find the true ‘me’. The first stage (session 6) is ‘differentiation from persona’, the second stage (sessions 7-8) is ‘consciousness of the shadow’. The third stage (sessions 9-13) is ‘fusion with anima’, and the fourth stage (sessions 14-15) is ‘self-actualization (individualization process)’. The client entered a world of light. With 3 months left until death, he integrated tconflicting things in the last 10th session of the sandbox. In this study, the client went to heaven. Through sand play counseling, the client’s anxiety and fear about death were alleviated, and he was able to accept death naturally and comfortably and organize his mind well. The client ended up with a “sacred marriage with death” as the wholeness of self-realization that had meaning.
