상하악 제1소구치 발치를 동반한 골격성 III급 부정교합의 절충 치료 치험례

Camouflage treatment of skeletal Class III malocclusion with extraction of bimaxillary first premolars

성장이 완료된 성인 골격성 III급 부정교합 환자의 치료에서 수술과 비수술을 결정하는 기준에 대해서 현재까지도 많은 논의가 있어왔다. Kerr 등은 측모두부방사선 분석상 기준치들을 제시하였고, Angelika 등은 이를 조합한 공식을 제시하기도 하였다. 일반적으로 전후방적 골격 부조화가 경미하고 단안모이며 연조직 외형이 양호하고 치성 보상이 적게 일어난 환자에서는 절충 치료만으로도 만족스러운 결과를 얻을 수 있다. 본 증례에서는 전치부 반대교합과 심한 총생을 동반한 성인 골격성 III급 부정교합 환자를 상하악 제1소구치 발치를 통해 절충 치료하여 교합과 안모의 심미성을 개선하였기에 이를 소개하고자 한다.

There have been many discussions on the criteria for surgical and non-surgical decision in the treatment of adult skeletal Class III malocclusion patients. Kerr et el. proposed several baseline cephalometric measurements, and Angelika et al. presented an equation that combined these measurements. In general, patients with mild anteroposterior skeletal disharmony, short face, acceptable soft tissue profile, and lesser dental compensation can be treated successfully with camouflage treatment only. This case report shows an adult with skeletal Class III malocclusion, anterior crossbite and severe crowding treated by camouflage treatment with bimaxillary first premolar extraction which resulted in good occlusion and improved facial profile aesthetics.
