상악 우측 대구치 매복을 동반한 골격성 III급 부정교합의 치험례

Treatment of skeletal Class III with impacted upper molars

치주인대는 교정력에 반응하여 골의 형성 및 개조(bone formation, remodeling)를 유도한다. 이같은 교정력에 의한 치아이동과 치주인대의 골 형성능은 외과적 술식으로 해결이 어려운 수직적 골 결손 상황에서 유용한 골 형성 방법이 될 수 있다. 본 증례는 상악 제1대구치 후방부의 수직적 골 결손이 심한 상황에서, 1차적 맹출 실패나 유착이 의심되는 미맹출 상악 제2대구치의 아탈구 및 교정적 견인을 통해 양호한 골 형성을 유도하였기에 이를 소개하고자 한다.

Periodontal ligament induces bone formation and remodeling in response to orthodontic force.1 Such orthodontic tooth movement and bone forming ability of periodontal ligaments can be a useful bone formation method in vertical bone defect situation which is difficult to be solved by surgical operation. In this case, we describe the induction of proper bone formation through subluxation and orthodontic traction of a non-erupted maxillary second molar caused by either primary eruption failure or ankylosis under severe vertical bone defect in the posterior part of the maxillary first molar.
