매복된 하악 제3대구치의 직립을 동반한 골격성 III급 부정교합의 절충 치료 치험례

Orthodontic camouflage treatment of skeletal Class III patient with third molar uprighting

성장이 완료된 골격성 III급 환자의 경우 골격적 부조화의 정도, 안모, 환자의 인식과 요구도 등을 고려하여, 절충 치료만으로 안모와 기능개선이 어려운 경우 악교정 수술을 계획하고, 골격적 부조화의 정도가 심하지 않다면 교정적 절충 치료를 계획해 볼 수 있다. 또한 구치 상실 시 치료계획으로는 자가이식, 보철적 수복, 제3대구치의 교정적 전방이동 등을 고려해 볼 수 있다. 본 증례에서는 상악 열성장을 동반한 골격성 III급 부정교합 환자에서 상하악 제1소구치 발치를 통한 절충 치료로 개선된 안모와 심미적인 미소를 얻을 수 있었으며, 골성 고정원 사용을 통하여 상악 우측 제1대구치 고정원 소실의 최소화와 하악 우측 제3대구치의 교정적 직립 및 전방이동을 통하여 추가적인 보철치료를 하지 않고도 양호한 치료 결과를 얻었기에 이를 소개하고자 한다.

In the case of non-growing patients with Class III skeletal malocclusions, degree of skeletal discrepancy, facial profile, patient’s perception and demand should be considered for treatment planning. In a patient with mild to moderate skeletal Class III malocclusions with acceptable facial profile, orthodontic camouflage treatment can be an alternative option whereas patients with severe skeletal discrepancy need orthognathic surgery to acquire proper function and facial esthetics. If the molar was lost, autogenous transplantation, implantation or protraction of third molar can be an treatment option as well. In this case, we present a skeletal Class III malocclusion patient with a retrognathic maxilla. By orthodontic camouflage treatment with premolar extraction, esthetic smile arc and harmonious facial profile were achieved. Orthodontic skeletal anchorages were used to upright lower right third molar and to reinforce anchor for upper right first molar. As a result, good treatment results were obtained without additional prosthodontic treatment.
