Simple and Efficient Treatment of Ectopic Eruption of Maxillary First Molars: Direct Single Nickel-Titanium Wire Technique

Early diagnosis and appropriate treatment are essential as ectopic eruption of first molars can cause problems such as root resorption, premature eruption of adjacent teeth, malocclusion and loss of space. In this case, a simple and efficient orthodontic technique was used to address the mesial tilting and ectopic eruption of the maxillary first molars by using nickel titanium (Ni-Ti) wire bonded directly to the tooth surface to move the first molars posteriorly, bypassing the conventional and complicated appliance fabrication process. This method used a thin 0.012-inch NiTi wire to generate a light, continuous orthodontic force to move the first molars posteriorly with minimal stress on the anchoring second premolars. By utilizing the high elasticity and low stiffness of the NiTi wire, side effects such as pain and root resorption were reduced, and the use of additional appliances such as brackets was not required to reduce patient discomfort and facilitate oral hygiene. In addition, by attaching the wire directly to the tooth surface, the problem of play associated with the use of brackets was eliminated, maximizing treatment efficiency and reducing treatment time. This is a case of effective treatment of first molars by displacing the posterior tilting of the crown, which shows that the simple and efficient treatment method using NiTi wires can be applied to various orthodontic treatments.
