노인장기요양 및 장애인 활동지원 서비스 비교를 통한 고령장애인 돌봄서비스 지원 연구: Gilbert & Specht 분석틀을 중심으로

A study on support for elderly disabled care services through comparison of elderly long-term care and Personal Assistance Service: Focusing on the Gilbert and Specht analytical framework

본 연구는 최근 변화(노인장기요양서비스의 인지지원등급, 장애인 서비스 종합지원조사 도입 등)가 반영된 ‘고령장애인 돌봄서비스’를 분석함으로써 돌봄안전망을 구축하고자 한다. Gilbert & Specht 정책분석틀을 중심으로 노인장기요양서비스와 장애인 활동지원 서비스의 할당체계, 급여체계, 전달체계, 재정체계 등을 비교분석하였다. 노인장기요양서비스는 재정 측면에서 보통 수준이었으나 할당‧급여‧전달체계에서는 전반적으로 미흡한 것으로 나타났다. 장애인 활동지원 서비스는 노인장기요양서비스에 비해 재정상태가 좋지 않지만 서비스의 질은 다소 높은 것으로 분석되었다. 고령장애인의 ‘돌봄 사각지대’ 및 ‘삶의 질 제고’를 위한 대안은 다음과 같다. 첫째, 돌봄 사각지대 해소를 위해 서비스 중복적용을 허용하거나 당사자에게 권한(선택권)을 부여하는 방안이 필요하다. 둘째, 고령장애인의 개별특성을 고려한 보장성 확대(방문간호급여 추가 등)가 필요하다. 셋째, 서비스 질 확보를 위해 당사자 및 다양한 집단에 권한을 부여하고 제공기관‧제공인력의 수준을 제고시켜야 한다. 넷째, 국가재원을 확보하여 본인부담금을 감소시킴으로써 지속적인 서비스 제공이 필요하다. 이와 더불어, 2026년 도입예정인 장애인 개인예산제도와 기존 정책 간 정합성에 대한 추후 논의도 필요하다.

This study aims to establish a care safety net by analyzing ‘care services for the elderly and disabled’ that reflect recent changes. Based on the Gilbert & Specht policy analysis framework, allocation system, payment system, delivery system, and financial system of long-term care services for the elderly and Personal Assistance Service were compared and analyzed. Long-term care services for elderly were at an average level in terms of finance, but were found to be generally inadequate in terms of allocation, payment, and delivery systems. Although the financial status of Personal Assistance Service is not as good as that of the elderly long-term care service, the quality of the service is analyzed to be somewhat higher. Alternatives for ‘blind spots in care’ and ‘improving the quality of life’ of elderly people with disabilities are as follows. First, in order to eliminate blind spots in care, it is necessary to allow overlapping application of services or grant authority to the parties concerned. Second, it is necessary to expand the coverage that takes into account the individual characteristics of the elderly and disabled. Third, in order to secure service quality, we must empower the parties and various groups and improve the level of service providers and personnel. Fourth, we must secure national funds to reduce out-of-pocket expenses, thereby ensuring continuous service provision. In addition, further discussion is needed on the consistency between the personal budget system for persons with disabilities scheduled to be introduced in 2026 and existing policies.
