정신의료기관 입원 중 경험하는 부정적 정서가 정신장애인의 회복에 미치는 영향 : 학대와 폭력으로부터 자유로운 서비스의 매개 효과 분석

The Impact of Negative Emotions Experienced During Psychiatric Hospitalization on the Recovery of Individuals with Psychosocial Disabilities: The Mediating Effect of Services Free from Abuse and Violence

우리나라는 장애인권리협약 비준 국가로서 정신장애인의 회복을 위해 학대와 폭력으로부터 자유로운 서비스 환경을 조성해야 할 의무가 있다. 그러나 장애인권리협약 비준 국가가 된 지 오랜 시간이 흘렀음에도 불구하고 정신장애인이 정신의료기관에서 두려움, 분노, 긴장 등의 부정적 정서를 경험하고, 이러한 부정적 정서가 당사자의 회복을 저해하는 요인으로 작용하고 있다는 당사자들의 보고가 있다. 이러한 배경에 따라, 본 연구에서는 정신질환을 경험하는 사람들이 입원 과정 혹은 입원 중에 느낀 부정적 정서가 회복에 주는 영향을 검증하고자 한다. 더불어 우리나라의 정신의료기관들이 장애인권리협약 비준 국가로서 갖춰야 할 학대와 폭력으로부터 자유로운 서비스가 매개 역할을 하는지 검토하고자 한다. 연구 문제를 살펴보기 위해, 본 연구에서는 2022년 정신장애인 인권친화적 치료환경 구축을 위한 실태조사에 참여한 정신장애인 348명 대상 데이터를 활용하여 구조방정식 분석과 간접효과 분석을 실시하였다. 분석 결과, 학대와 폭력으로부터 자유로운 서비스는 완전 매개 효과를 갖는 것으로 확인되었다. 이러한 연구 결과에 기반하여 정신의료기관의 입원 환경을 학대와 폭력으로부터 자유롭게 조성하기 위한 실천 및 정책적 제언을 하였다.

South Korea, as a ratified party to the Convention on the Rights of Persons with Disabilities (CRPD), has an obligation to create a service environment free from abuse and violence that supports the recovery of individuals experiencing psychosocial disabilities. Despite significant time having passed since ratifying the CRPD, many individuals with psychosocial disabilities continue to report negative emotions such as fear, anger, and tension during hospitalization or the admission process. These negative emotions have been reported as factors hindering recovery. In light of this, the present study aims to examine the impact of negative emotions experienced during hospitalization or the admission process on recovery. Furthermore, it explores whether services free from abuse and violence, essential in psychiatric hospitals under the CRPD framework, mediate this relationship. To examine the research question, this study utilized data from 348 individuals with psychosocial disabilities who participated in the 2022 survey on building a human rights-friendly treatment environment. Structural equation modeling and mediation analysis were conducted. The results indicated that freedom from abuse and violence in services had a full mediation effect. Based on these findings, practical and policy recommendations were made to create an inpatient environment in mental health institutions that is free from abuse and violence.
