Integrative Medical Social Work: Theories and Practice, focusing on the Collaborative Care Model

통합적 의료사회복지: 이론과 실제, 협력적 돌봄 모델을 중심으로

This paper examines the theories and practices of integrative medical social work with a particular emphasis on the collaborative care model (CCM). The CCM promotes patient-centered, population-based, measurement-guided, evidence-informed and accountable care. Anchored in the Grand Challenges for Social Work initiative, particularly the Close the Health Gap challenge, this study investigates innovative strategies for embedding social care within healthcare systems. Practical application is demonstrated through an institutional example (Integrated Health Hawaii, LLC) and an in-depth patient case study. This paper provides both a theoretical framework and practical insights for advancing medical social work, offering implications for academic and clinical practice, particularly within the South Korean context.

이 연구의 목적은 보건 복지 현장에서 사회적 케어의 통합(Integrating Social Care in Health Care Setting)을 중심으로 통합적 의료사회복지(Integrative Medical Social Work)의 이론과 실천을 소개하는 것이다. 구체적으로 환자 중심, 인구 기반, 측정 중심, 증거 기반, 책임 있는 돌봄을 강조하는 협력적 돌봄 모델(Collaborative Care Model, CCM)과 같은 증거 기반 실천을 강조하였다. 협력적 돌봄 모델의 실천 환경에의 적용을 위하여 첫째, 특정 기관의 예시(Integrated Health Hawaii: IHH, LLC)를 소개하고 둘째, 환자 사례를 제시하였다. 본 연구가 한국 의료사회복지의 이론적 근거와 실천에 기초자료로 활용될 수 있기를 기대한다.
