This case report describes the treatment of sibling patients with skeletal Class III malocclusion. The brothers received compromised treatment and surgical-orthodontic treatment, respectively. The younger brother, JS-c started his compromised treatment at the age of 16 years and 6 months; the elder brother, JS-s, who had undergone surgical-orthodontic correction, started treatment 11 months later than his younger brother, when he was 19 years and 2 months old. Before treatment, both patients exhibited mandibular protrusion and maxillary retrusion, but the elder brother, JS-s had more concave profile. In JS-s, a compensation of dentition due to skeletal discrepancies was greater than that of JS-c. In JS-c, after 36 months of treatment, Class I canine and molar relationship was attained after moving entire mandibular dentition backward, but the skeletal class III relationship was further strengthened with significant growth of the mandible during treatment. In JS-s, after 28 months of treatment including orthognathic surgery, a skeletal and dental Class I relationship was achieved, with a remarkable improvement of facial profile.
