Orthodontic Treatment of Skeletal Class II, Division 2 Malocclusion with Bilateral Second Molar Scissors Bite

This case report describes an orthodontic treatment of a 37-year-11-month-old female patient who had a skeletal Class II, Division 2 malocclusion with bilateral second molar scissors bite. She also showed crowding, gummy smile, and protrusive lip profile. To correct the posterior scissors bite, modified transpalatal arch and anterior bite plane were used. Distalization of the maxillary posterior segments was performed to correct Class II dentition with two posterior microimplants installed between the maxillary second premolar and first molar on both sides. Overerupted maxillary incisors and canines were intruded with high-pull J hook (HPJH) headgear and the maxillary incisors were retracted with closing loops. The mandibular anterior teeth were intruded with conventional methods such as anterior bite plane, overlay wire, and Class II elastics. After orthodontic treatment, the mandible moved slightly forward with correction of the deep bite and intrusion of the maxillary incisors. This case report presents a successful treatment with directional force technique using microimplant anchorage and HPJH headgear.
