Orthodontic practitioners frequently encounter a spectrum of congenital dental anomalies, among which congenital absence of the permanent teeth are one of the most common congenital anomalies. Notably, maxillary lateral incisors emerge as the second most prevalent congenitally absent teeth. Treatment modalities to address these defects include three main options: canine substitution, tooth-supported restoration, and single-tooth implant. An important consideration when pursuing an individualized treatment strategy is to encompass the size, shape, and color shade of the canine selected for lateral incisor replacement in addition to existing orthodontic parameters. The optimal treatment path entails a wise balance between conservation of the teeth and meeting the patient’s aesthetic and functional requirements. Here, I report a case of an adolescent female patient with bilateral congenital missing of the maxillary lateral incisors, that I chose canine substitution as her preferred treatment option.
