A 15-year and 9-month-old male patient came for orthodontic consultation to solve his difficulty chewing problem. He had a skeletal Class III malocclusion with facial asymmetry of leftward mandibular deviation complicated by an anterior crossbite, an asymmetric maxillary arch, mesially tilted mandibular molars, and compensatory uprighting of the mandibular incisors. In adolescent patients with skeletal Class III malocclusion accompanied by facial asymmetry, the optimal treatment method is generally considered to be orthodontic treatment combined with orthognathic surgery a$er growth has ceased, to achieve better esthetic and functional outcomes. In this case, non-surgical orthodontic treatment was performed to alleviate severe discomfort from occlusal interference, taking into account the potential for further growth and the potential limitations in achieving the final treatment objective in terms of the magnitude of facial improvement. As the mandibular growth progressed during the treatment period, the treatment was terminated after confirmed his occlusal stability. It took a total of four years. Despite additional mandibular growth, the post-retention results maintained satisfactorily over a 9-year followup period. This case report outlines the diagnostic and treatment considerations that facilitated highly effective and stable treatment outcomes for non-surgical orthodontic treatment in an adolescent with skeletal Class III malocclusion.
