심한 하악 전돌을 보이는 환자의 하악골 전방부 분절골 절제술을 동반한 수술교정 치험례

Surgico-orthodontic treatment with mandibular anterior segmental osteotomy in a patient with severe mandibular prognathism

목적: 심한 골격성 III급 부정교합 환자에서 하악 전돌 양상 해결을 위해 상악 Le Fort I 골절단술, 하악지 수직골절단술(IVRO) 및 하악 전방부 분절골 절제술(ASO)을 동시에 진행하여 안정적인 기능과 교합, 심미적인 측모를 형성한 증례를 보고하고자 한다. 증례: 23세 여성 환자가 아래턱 돌출 해소를 위한 악교정 수술 전 교정치료를 주소로 구강악안면외과에서 의뢰되어 내원하였다. 임상 및 방사선 평가를 통해 안면비대칭과 횡적 부조화를 동반한 골격성 III급 부정교합으로 진단하였다. 심미적인 측모와 술후 안정성을 고려하여 하악 전방부 근첨하 골절단술, Le Fort I 골절단술과 IVRO를 동시에 시행하였다. 수술 전 치료 계획에 비해 ASO 골절 단선 부위의 공간이 과도하게 남아 술후 교정 시 폐쇄하였으며, 적절한 교합관계와 심미적인 안모를 달성하였다. 결론: 심한 골격성 III급 부정교합 환자에서 적절한 교합 관계 형성을 위해 두 차례 이상의 악교정 수술이 필요할 수 있으나 본 증례에서는 IVRO와 ASO를 동시 시행함으로써 술후 안정성 및 환자의 편의를 도모했으며 수술적 한계 등을 술후 교정치료를 통해 해결하여 이를 소개하고자 한다.

Purpose: This study aimed to report a case of single-step surgery including Le Fort I osteotomy, intraoral vertical ramus osteotomy (IVRO), and anterior segmental osteotomy (ASO) of the mandible, performed to resolve mandibular protrusion in a patient with severe skeletal Class III malocclusion. The treatment achieved long-term stability in functional occlusion and improved the lateral facial esthetics. Case Report: A 23-year-old woman was referred from the Department of Oral and Maxillofacial Surgery for orthodontic treatment prior to orthognathic surgery for mandibular protrusion. Clinical and radiographic evaluations revealed skeletal Class III malocclusion with facial asymmetry and transverse discrepancy. Considering the aesthetic profile and postoperative stability, anterior segmental osteotomy of the mandible was combined with Le Fort I osteotomy and IVRO at the same time. Postoperative orthodontic treatment involved closure of the excessive residual space at the osteotomy site achieving an appropriate occlusal relationship and aesthetic facial appearance. Conclusion: Multiple-step orthognathic surgery may be necessary to achieve a proper occlusal relationship in patients with severe skeletal Class III malocclusion. However, in this case, IVRO and ASO were simultaneously performed, enhancing postsurgical stability and patient convenience. This case demonstrates the successful management of postsurgical complications through orthodontic treatment.
