상악 제1대구치 결손과 정준선 편위를 동반한 환자의 마이크로 임플란트를 이용한 비발치 교정치료

Micro-implant assisted non-extraction treatment of a patient with missing maxillary first molar and midline discrepancy

목적: 상악 제1대구치 결손과 정중선 편위를 보이는 환자에서 마이크로 임플란트를 이용해 상악 양측의 비대칭적 치아이동을 통해 정중선 편위를 개선하고 보철 공간을 마련한 증례를 보고하고자 한다. 증례: 본 19세 여성은 치아 사이 공간을 주소로 본원에 내원하였으며, 골격성 I급 부정교합, 상악 우측 제1대구치의 결손과 정중선 편위를 보였다. 상·하악의 정중선을 안모 정중선에 일치시키기 위해 마이크로 임플란트를 이용해 상악 우측 구치부의 전방이동과 상악 좌측 구치부의 후방이동, 하악 우측 후방이동을 시행하였다. 그 결과 정중선 편위가 개선되었으며 상악 우측 최후방 구치부에 임플란트 식립을 위한 공간을 성공적으로 마련하였다. 결론: 편측성 결손치로 인해 정중선 편위를 보이는 환자에서 마이크로 임플란트를 이용한 치아의 치체 이동을 통해 치아의 정중선을 안모 정중선에 일치시키고 심미성을 개선하였다.

Purpose: This case report presents the correction of midline deviation and creation of prosthetic space in a patient with a missing maxillary first molar through asymmetric tooth movement of the bilateral maxillary dentition using micro-implants. Case Report: A 19-year-old woman presented to our clinic with a chief complaint of spacing between teeth. Clinical examination revealed skeletal Class I malocclusion, absence of the right maxillary first molar, and midline deviation. To align the maxillary and mandibular midlines with the facial midline, micro-implants were used for mesialization of the right maxillary posterior teeth, distalization of the left maxillary posterior teeth, and distalization of the right mandibular teeth. Consequently, the midline deviation was successfully corrected, and adequate space was created in the right maxillary posterior region for implant placement. Conclusion: Bodily tooth movement using micro-implants successfully aligned the dental midline with the facial midline in a patient with midline deviation due to unilateral tooth loss, thereby improving aesthetic outcomes.
