Hyperdivergent skeletal Class II malocclusion is characterized by a combination of sagittal and vertical skeletal discrepancies, making treatment particularly challenging. With the advancement of skeletal anchorage, molar intrusion has become a predictable treatment option, enabling esthetic outcomes for cases with mild to moderate skeletal discrepancies. A 23-year-old male patient presented with lip incompetence and mandibular retrusion. The patient exhibited a skeletal Class II relationship with a hyperdivergent pattern. The treatment strategy involved extraction of four premolars and intrusion of the maxillary posterior teeth, promoting counterclockwise rotation of the mandible to enhance the soft tissue profile. In camouflagetreatment of hyperdivergent skeletal Class II malocclusion with molar intrusion, careful consideration of multiple factors is required to maximize the reductionof vertical dimension. This approach can lead to favorable outcomes in vertical, sagittal, and esthetic aspects.
