Orthodontic Treatment of an Adult Patient with Unilateral Extraction and Impacted Maxillary Central Incisor

Impaction of the maxillary central incisor is relatively low at less than 2% but it can lead to significant esthetic and functional concerns. The primary cause of maxillary central incisor impaction is the obstrusction of the eruption pathway, often due to factors such as supernumerary tooth or odontoma. This case report presents a 19-year-old female with an unerupted left maxillary central incisor associated with a compound odontoma and skeletal Class II malocclusion. To avoid damaging adjacent teeth and to ensure eruption through the attached gingiva, initial traction force was applied in the palatal direction using a customized palatal traction device. As a result, the impacted incisor erupted successfully at the appropriate gingival level without recession, achieving favorable esthetic and functional outcomes.
