Non-extraction Orthodontic Treatment Following Lingual Myoplasty in an Adolescent Patient with Lip Protrusion and Anterior Diastema

Tongue thrusting is a detrimental oral habit that contributes to various malocclusions, including anterior open bite, diastema, and bialveolar protrusion. This case report details the successful management of a Class I malocclusion with anterior diastema and lip protrusion in an 11-year-old male patient. Treatment involved a non-extraction orthodontic approach following a lingual myoplasty (LMP) procedure combined with targeted muscle training. Three years post-LMP and muscle rehabilitation, the patient exhibited marked improvements in lip posture and dental alignment, allowing for orthodontic correction without premolar extraction. The treatment utilized removable appliances equipped with expansion screws and two mini-implant anchorage systems to facilitate posterior movement of the entire dentition. As a result, the Z-angle increased from 60° to 74.5°, enhancing facial balance and achieving optimal Tweed triangle parameters.
