프로이트의 히스테리 환자 에미 폰 N. 부인에 대한 정신분석적 임상 사례 연구

A Psychoanalytic Clinical Case Study of Mrs. Emmy von N., One of Freud’s Hysterical Patients

본 논문은 히스테리 환자들 가운데 프로이트(S. Freud)가 카타르시스 요법을 적용하여 최면으로 히스테리 증상을 치료한 첫 번째 임상 사례인 에미 폰 부인(Mrs. Emmy von N.)에 대한 정신분석적 연구이다. 연구자는 에미 폰 부인의 성장 과정 및 심리 상태, 에미 폰 부인의 히스테리 발달 과정으로서의 억압과 정동, 그리고 에미 폰 부인의 히스테리 치료과정에 관해 고찰하였다. 그 후 에미 폰 부인의 억압된 성적인 욕망과 히스테리와의 관계, 에미 폰 부인의 히스테리 치료를 위한 카타르시스 요법으로서의 최면 치료와 그 한계를 연구하였다. 마지막으로 에미 폰 부인에 관한 임상 사례 연구의 현대적 의미에 대하여 ‘오이디푸스 콤플렉스’의 기초가 되는 양가감정으로서의 ‘콤플렉스’ 의미의 발견, 정신분석적 치료에 있어서 전이의 개념 형성의 토대가 된 연구로서의 가치, 자유연상 기법의 발견 및 활용의 토대가 된 연구로서의 가치, 그리고 신체적 접촉이 없는 정신분석 치료법으로의 전환의 필요성에 대해 제시하였다.

This paper presents a psychoanalytic study of Mrs. Emmy von N., widely regarded as the earliest documented case in which Freud employed the cathartic method under hypnosis to treat hysterical symptoms. This study examines her developmental history and psychological condition; the roles of repression and affect in the emergence of her hysteria; and the clinical course of her treatment. It further examines the relation between Mrs. Emmy von N.’s suppressed sexual desires and hysteria; evaluates hypnotic treatment used as the cathartic method in treating her hysteria; and delineates the limitations of this approach. Finally, it articulates the contemporary significance of this clinical case study of Mrs. Emmy von N.: the early use of the concept of the ‘complex,’ which later underpinned the Oedipus complex; the study’s contribution to the formation of the concept of transference in psychoanalytic treatment; its role in laying the groundwork for the discovery and application of the free-association technique; and the need for a transition toward psychoanalysis conducted without physical contact.
