입술의 부조화와 돌출을 동반한 골격성 II급 노년 환자에서 하악골의 반시계방향 회전을 통한 안모개선 치험례

Facial profile improvement by counter-clockwise mandibular rotation in an elderly patient with skeletal Class II malocclusion with lip incompetency and protrusion

목적: 입술의 부조화와 돌출을 동반한 골격성 II급 노년 환자에서 미니스크류를 이용한 상악 전치열의 압하 및 하악골의 반시계방향 회전으로 안모를 개선한 증례를 보고하고자 한다. 증례: 본 59세 여환은 골격성 II급으로서 SN-GoMe 45.9°의 발산형 안모, 입술의 돌출, 상·하악 전치부의 경미한 총생을 보였다. 상악 협측 제1소구치와 제2소구치 사이, 제2소구치와 제1대구치 사이에 총 4개의 미니스크류를 식립하여 상악 전치열을 후상방으로 압하하였고, 하악에도 두 개의 미니스크류를 식립하여 치아의 정출을 방지하고 하악골의 반시계방향 회전량을 최대화하였다. 환자 짧아진 전안면 고경과 전진한 이부로 인하여 개선된 안모에 만족하였다. 결론: 입술의 부조화와 돌출 및 발산형 안모를 동반한 환자에서 미니스크류를 고정원으로 사용하여 하악의 반시계방향 회전을 통해 안모를 개선하였고, 입술의 편안함을 확보하였다.

Purpose: This case report presents the improvement in facial appearance in an elderly patient with skeletal Class II malocclusion with lip imbalance and protrusion through the use of miniscrews for maxillary total arch intrusion and counter-clockwise mandibular rotation. Case Report: A 59-year-old woman with skeletal Class II malocclusion presented with a hyperdivergent facial profile (SN-GoMe 45.9°), lip protrusion, and mild crowding of both maxillary and mandibular anterior teeth. Four mini-screws were placed between the maxillary first and second premolars and between the second premolar and first molar to intrude the maxillary arch in the posterior and superior directions. Additionally, two mini-screws were placed in the mandible to prevent extrusion of the arch and maximize the counterclockwise mandibular rotation. The patient was satisfied with the improved facial appearance, including reduced anterior facial height and a more forward-positioned chin. Conclusion: We used mini-screws as an anchorage to effectively induce counterclockwise mandibular rotation and improve facial appearance and lip comfort in a patient with lip incompetency, protrusion, and a hyperdivergent facial profile.
