Secondary rhinoplasty for unilateral cleft nasal deformity remains challenging due to alar base asymmetry, collapse of the lower lateral cartilage, and depression of the nasal base. Achieving long-term nasal symmetry and structural stability continues to be a primary reconstructive objective. This report presents three cases of secondary unilateral cleft rhinoplasty performed using a bipedicled lower lateral cartilage flap. The technique preserves the continuity of the lower lateral cartilage through both medial and lateral pedicles, thereby providing enhanced alar dome support and improved nasal tip projection. All patients had previously undergone primary cleft lip repair and exhibited nasal asymmetry with alar collapse. The bipedicled flap was elevated and repositioned to restore nasal contour without the need for additional cartilage grafts from other donor sites. Postoperative outcomes demonstrated improved nasal symmetry, stable tip projection, and satisfactory aesthetic results, with no major complications observed.
