This case report presents the successful substitution of severely resorbed maxillary central incisors with bilaterally impacted canines, using a comprehensive surgical-orthodontic approach. Radiographic assessment confirmed the extent of root resorption and precise positions of the impacted canines. Treatment began with extraction of the compromised central incisors followed by orthodontic traction to reposition the impacted canines into the central incisor positions. The first premolars were subsequently moved to the canine positions, and prosthetic restoration of the substituted canines was done to achieve optimal anterior aesthetics and simulate natural central incisor morphology. Posttreatment evaluation revealed stable occlusal relationships and harmonious smile. The 5-year follow-up examination confirmed maintained periodontal health, stable occlusion, and enduring aesthetic outcomes, validating the long-term success of this multidisciplinary treatment approach. This case demonstrates that canine substitution, when properly executed with appropriate orthodontic and prosthetic protocols, can serve as a viable treatment option in cases of severe central incisor compromise due to root resorption caused by impacted canines.
