In non-extraction camouflage treatment of Class II malocclusion, maxillary molar distalization is often required. Such movements can impact both vertical and transverse occlusal relationships. Therefore, the primary goals are to minimize side effects and to achieve stable molar distalization. This paper demonstrates that successful distalization of maxillary molars can be achieved by simultaneously applying forces from both buccal and palatal sides using the Biocreative molar appliance (BMA) and skeletal anchorage devices. This technique allows for the concurrent push on both the first and second molars, enabling a substantial amount of distalization in a relatively short period of time. Compared to unilateral approaches, it also prevents molar rotation and thus better preserves the original arch form. To ensure predictable outcomes, careful consideration must be given to the type and placement of the palatal skeletal anchorage, as well as the specific design of the transpalatal arch.
