국가보훈대상자의 의료서비스 이용 영향요인에 관한 연구: 선행요인, 가능요인, 욕구요인, 정책요인을 중심으로

A Study on the Factors Affecting Medical Service Use of National Veterans: Focusing on Predisposing Factors, Enabling Factors, Need Factors, Policy Factors

본 연구의 목적은 중앙보훈병원을 이용하는 국가보훈대상자의 의료서비스 이용에 영향을 미치는 요인을 분석하고 이를 통해 이론적, 정책적 시사점을 제시하는 것이다. 앤더슨 모형에 대한 검토와 확장을 통해 국가보훈대상자의 의료서비스 이용을 외래진료 이용과 입원진료 이용으로 구분하고, 예측요인인 선행요인, 가능요인, 욕구요인, 정책요인과의 관계에 관한 가설을 설정하였다. 중앙보훈병원을 이용하는 국가보훈대상자 352명을 대상으로 설문조사를 실시하였으며, 수집된 자료를 활용하여 다중회귀분석을 통한 가설검증을 시도하였다. 가설검증 결과에 따르면, 국가보훈대상자의 외래진료 이용에는 국가보훈대상자의 연령, 거주지역, 의료보장 형태, 주관적 건강 상태와 만성질환 수, 그리고 경제적 의료비 지원이 유의한 영향을 미치는 것으로 분석되었다. 입원진료 이용에는 성, 월평균 가구소득, 만성질환 수, 장애 정도, 그리고 지각된 의료지원 정책의 충분성이 유의한 영향을 미치는 것을 확인할 수 있었다. 이러한 분석 결과를 토대로 국가보훈대상자를 위한 효과적인 의료서비스 지원방안에 대해 논의하였다.

The purpose of this study is to analyze the factors affecting the utilization of healthcare services by National Merit Subjects using the Central Veterans Hospital and to suggest theoretical and policy implications. Through a review of prior research, we extended the Andersen Model and differentiated the healthcare service utilization of National Merit Subjects into outpatient service utilization and inpatient service utilization. Subsequently, we developed hypotheses by setting the predicting factors for healthcare service use as predisposing factors, enabling factors, need factors, and policy factors. To test the hypotheses, a survey was conducted on 352 National Merit Subjects utilizing the Central Veterans Hospital. The collected data were analyzed using multiple regression analysis for hypothesis testing. The results showed that the outpatient service utilization of National Merit Subjects is significantly influenced by their age, residential area, medical aid status, subjective health status and number of chronic diseases, and national financial support for medical expenses. Furthermore, the results indicated that inpatient service utilization was significantly affected by gender, average monthly household income, number of chronic diseases, degree of disability, and perceived sufficiency of medical support. Based on these results, this study discussed effective healthcare service support strategies and proposals for National Merit Subjects.
