Challenges and Operational Status of the Home Health Care Program: A Visiting Nurse-Centered Study

Purpose: This study examined the operational status and challenges of a public health center-based home healthcare program in Seoul from the perspective of visiting nurses and explored the implications for integrated community care. Methods: A mixed-methods design was used with secondary data collected by the Seoul Metropolitan Government in 2018. Quantitative survey data from 324 visiting nurses were analyzed using descriptive statistics, and qualitative open-ended responses were analyzed using content analysis based on Patton’s framework. The analysis was guided by the following five components of the national healthcare system: resource development, service delivery, economic support, management, and outcomes. Results: Visiting nurses were mainly middle-aged women working alone in community centers. The major challenges include insufficient field-oriented training, dual data entry across health and welfare information systems, limited collaboration with social workers, frequent visit refusals by older adults, and low wages with unstable employment conditions. Nevertheless, the nurses perceived the program as meaningful for the early identification of health and psychosocial needs. Conclusion: Structural barriers hinder IC effectiveness. Improving information system integration, workforce support, training, and compensation is essential for strengthening home-based nursing services and promoting the implementation of the Integrated Community Care Act in Korea.
