The purpose of this study was to investigate the effect of sling exercise therapy (SET) on balance, hand grip strength and muscle tension in chronic stroke patients. The SET program for strengthening trunk, pelvis, the upper and lower limb muscle function was applied to 15 patients with chronic stroke. The SET was applied for 10 minutes of warm-up exercise, 40 minutes of main exercise, and 10 minutes of cool down exercise. It was applied twice a week, for 5 weeks. Research tools include functional reach test (FRT), sit-to-stand test (STS), and hand grip strength. The frequency of the tibialis anterior, peroneus longus, gastrocnemius, stiffness (S), and elasticity (D) were measured using a muscle tone tester. As a paired-sample t-test was condicted. The significance level was set to p=0.05. As a result of the study, STS (sec) and FRT (cm) were significant (p<0.05). The hand grip strength (kg) was significantly increased (p<0.05). The muscle tension (F) had no significant effect on the patient and tendon side in all of the anterior tibialis anterior, peroneus longus, gastrocnemius (p>0.05). In the change of biomechanical properties, both tension and paresis side were significantly reduced in item D of the tibialis anterior (p<0.05), significant decrease in item S of gastrocnemius (p<0.05), and significant decrease in item S of gastrocnemius(p<0.05). In chronic stroke patients, muscle endurance strengthening exercise using sling showed significant results in improving balance and muscle strength, and positive results were also found in the biomechanical properties (D, S) of tibialis anterior, peroneus longus, gastrocnemius.
