Exploring Directions for Developing Narrative Care Competence as a Medical Professionalism

현대 의료환경은 질병 중심의 생의학적 모델에서 벗어나 환자의 삶과 경험을 반영하는 생물정신사회적 모델로의 전환을 요구하고 있다. 이러한 변화 속에서 환자의 이야기를 경청·이해하고, 이를 진료에 반영하는 내러티브 케어 역량은 의료전문성의 핵심 요소로 주목받고 있다. 본 연구는 문헌 분석과 관련 사례 검토를 통해 내러티브 케어 역량의 개념과 필요성을 규명하고, 이를 의료전 문성 개발에 접목하기 위한 교육적·실천적·제도적 방향을 제시하였다. 내러티브 케어는 치료적·윤리적·교육적 차원을 포괄하며, 환자의 전인적 회복, 인간 존엄성의 회복, 정체성과 공동체 형성을 지원하는 통합적 돌봄 접근으로 정의된다. 본 연구는 내러티브 케어 역량의 구체적 요소로 환자 서사 중심 진료, 질병 경험 기반 진단, 자기 성찰, 감정 관리, 환자 서사 활용, 맥락 기반 결정, 윤리 실천, 인문 감수성 등 여덟 가지를 도출하였다. 교육적으로는 의과대학 전 교육과정에 내러티브 생성·공유·성찰 단계를 구조화하여 공감 능력, 윤리적 판단, 전문직 정체성을 체계적으로 함양할 것을 제안하였다. 실천적으로는 환자와의 대화 능력 향상, 서사 기록 시스템 구축, 피드백 체계 운영을 통해 진료 현장에 적용할 수 있는 기반을 마련하고자 하였다. 제도적으로는 평생교육 체계 확립, 전문 교육자 양성, 교육 참여 인센티브 제공 등 지속 가능한 지원 방안을 강조하였다. 이러한다층적 접근은 기술 중심 진료에서 인간 중심 진료로의 전환을 촉진하고, 환자 중심 의료서비스의 질 향상에 기여할 수 있으며, 향후 다양한 의료 환경에서 내러티브 케어 역량의 적용 가능성과 제도화를 검증하는 실증 연구가 필요하다.

The transition from a biomedical model to a biopsychosocial approach underscores the need to integrate patients’ narratives into healthcare. Narrative care competence—the ability to listen to, understand, and apply patient stories—has become a core element of medical professionalism. This study, through literature review and case analysis, explores the concept and significance of narrative care and proposes strategies for its development. Narrative care embraces therapeutic, ethical, and educational perspectives, supporting recovery, human dignity, and identity formation. Eight key components were identified: patient-narrative-centered practice, illness-experience-based diagnostics, self-reflection, emotional regulation, narrative utilization, context-based decision making, ethical practice, and humanistic sensitivity. Educationally, structured stages of narrative generation, sharing, and reflection should be embedded in curricula. Practically, communication skills, documentation systems, and feedback mechanisms are essential. Institutionally, lifelong learning, educator training, and incentives are required. Such a multi-layered approach fosters a shift from technology-centered to human-centered care, enhancing patient-centered medical services. Future research should empirically test the effectiveness of narrative care competence across healthcare contexts.
