This case report aims to highlight the management of suspected ankylosed teeth with surgical interventions and to discuss associated complications, such as cervical root resorption, through long-term follow-up. A 12-year-old Korean female presented with a posterior open bite and multiple teeth suspected of ankylosis, including infraoccluded teeth. Over 17 years, treatment included surgical replantation, and corticotomy to reposition and align the affected teeth. Surgical replantation of the mandibular first molars facilitated alignment, but progressive cervical root resorption subsequently developed, resulting in eventual extraction and implant placement. In contrast, the maxillary molar treated with corticotomy exhibited stable outcomes witho ut root resorption or pulp pathology. At the eight-year post-treatment follow-up, peri-implant bone levels remained stable, and occlusal function was successfully maintained. Surgical interventions, such as replantation and corticotomy, can be effective treatment options for managing ankylosed teeth and achieving functional and esthetic goals. However, complications such as cervical root resorption represent significant risks, emphasizing the need for thorough preoperative risk assessment, informed patient consent, and long-term follow-up. Regular radiographic evaluations is essential for the early detection and management of post-surgical complications to optimize treatment outcomes.
